An unknown quantity of left-over Second Edition sleeves were combined with Third Edition disks and the 1984 copyright notice insert, with a differently-formatted mechanical numbering system on the back sleeve: "990###". (The example in Images is stamped 990069, or #69 of this version.) The folded insert is the same as other editions.
